Youth/Teen Participation Consent Form

Your youth’s safety, well-being, and spiritual growth are our top priorities. We’re truly grateful for the opportunity to walk alongside them and share the Gospel through fellowship and community.

Please complete this consent form to help us serve your youth well. It includes essential information such as food allergies, medical needs, and emergency contacts. During events, we may serve snacks, fruit trays, or cupcakes for special occasions — and we want to make sure every child is cared for properly.

This consent form covers regular church fellowship activities and special events, including but not limited to:

Vacation Bible School (VBS)
Sunday Services & Sunday School
Community Outreach Events
Youth Group Gatherings and Celebrations
