AGREEMENT OF RELEASE AND WAIVER OF LIABILITY FOR HOT HULA FITNESS® and POLYNESIAN DANCE CLASSES

I acknowledge that I have voluntarily applied to participate in the Anna-Rita Sloss Enterprises, LLC, HOT HULA fitness® Class and Polynesian Dance classes with Nenette Alejandria Mayor.

I AM AWARE THAT PHYSICAL TRAINING ACTIVITIES CAN CAUSE INJURIES AND THAT I COULD BE INJURED. I AM VOUNTARILY PARTICIPATING IN THESE ACTIVITIES WITH KNOWLEDGE OF WHAT IS INVOLVED, AND AGREE TO ASSUME ANY AND ALL RISKS OF BODILY INJURY, WHETHER THOSE RISKS ARE KNOWN OR UNKNOWN.

As consideration for being permitted by Anna-Rita Sloss Enterprises, LLC, Drop In Dance Winnipeg, Nenette Alejandria Mayor, and their respective directors, officers, employees, volunteers, agents, contractors, and representatives (collectively “releasees”) from any and all actions, claims, or demands that I, my assignees, heirs, distributees, guardians, next of kin, spouse, and legal representatives now have, and/or may have in the future for injury related to my participation in these activities.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILTY AND A CONTRACT BETWEEN MYSELF AND ANNA-RITA SLOSS ENTERPRISES, LLC, DROP IN DANCE WINNIPEG, NENETTE ALEJANDRIA MAYOR, AND SIGN IT OF MY OWN FREE WILL.
