    I, the undersigned, as the parent/guardian of the above-named participant, give my full consent for my child to participate in the Teen Beauty Bootcamp organized by Glambox University. I understand that the activities will include workshops on hair braiding, makeup, skincare, and nail basics.

    In the event of an emergency, I hereby authorize the staff of Glambox University to secure any necessary treatment for my child. I also agree to be responsible for any costs associated with such treatment.

    I hereby release and hold harmless Glambox University, its staff, volunteers, and associates from any and all claims arising out of my child’s participation in the Teen Beauty Bootcamp.
