FITNESS TO PARTICIPATE: I, the legal guardian acknowledges to MASC Badminton Academy that the Participant does not have any physical limitations, medical ailments, or physical or mental disabilities that would limit or prevent the Participant from participating in the Activity. If required, the Participant will obtain a medical examination and clearance.

THIS LIABILITY WAIVER FORM is in consideration of being allowed to participate in the Activity and other good and valuable consideration, the receipt of which is hereby acknowledged, I (the "Legal Guardian"), agree on behalf of my child/children (the "Participant") with organizer MASC Badminton Academy Ottawa (MASC BAO)/Activity Provider and Ottawa Carleton District School Board/Conseil des ecoles publiques de l'est de l'ontario) to the following Minor Liability Waiver form.
