  • 1. Basic information

    Please fill out the below information.
  • Overview of the Safety Recognition Application Process

  • Section 1 details a company’s trailing indicators. A trailing indicator is typically based on injury and incident data (i.e., things that have occurred in the past). The trailing indicators used for an AHGP Safety Award are based on OSHA incident rates: the Total Recordable Case (TRC) Rate and the Days Away and Restricted Duty/Job Transfer (DART) case rate.

    The applicant’s TRC and DART rates must be at or below the most recent published BLS rates for “Specialty Contractors” to earn points. The TRC and DART case rates are each worth 12.5 points. Canadian firms participating in the AGHP Safety Recognition Program should use the Canadian Province Workers Compensation Law and/or Occupational Safety and Health Regulations as described below. Fill in the appropriate table for your company.

    Section 2 details leading indicators. Leading indicators are those techniques that your company uses to PREVENT rope access injuries/incidents. Leading indicators include: inspections, training, slope site assessments, accountable activities, etc. Section 2 contains 16 questions, each worth 5 points for a possible maximum of 80 points, or 75% of the Total Score.

    Section 3 contains the required narratives that allow each company to describe the critical elements of their Rope Access Safety program. Be sure to detail those elements of your program that have proven effective in preventing injuries and incidents.

    Section 4 contains the summary of your score.

    Section 5 contains the required signature and certificate of release page.

  • Section 1: Incident Rates for U.S. Companies

    Please answer the questions to the best of your ability.
  • The calculated TRC (Total Recordable Case Rate) and DART (Days Away, Restricted Duty, and Job Transfer Case) Rate for the calendar year 2023 are the criteria for this Section.

    To earn points, the applicant company’s TRC & DART rates for 2023 must be at or below published incident rates, as follows:

    Note: Please list your full company TRC & Dart rates for all work done (rope access and other). If you would like to apply for the Safety Recognition for a rope access division within your company, please note this in section 1.

    • A TRC rate at or below the published in the 2019 Bureau of Labor Statistics data for Construction Specialty Trade Contractors of 3.1 equals 12.5 points (TRC above the published rate = zero points).
    • A DART rate at or below the published 2019 Bureau of Labor Statistics for Construction Specialty Trade Contractors of 1.8 equals 12.5 points (DART above the published rate = zero points).
      • Note: Please list your full company Dart rate for all work done (rope access and other). If you would like to apply for the Safety Recognition for a rope access division within your company, please note so in section 1.

  • The TRC Rate and the Dart Rate will be calculated by the AGHP as per the following examples

    Examples:
    TRC 5 total OSHA recordables & 465,000 employee hours worked
    TRC = (5 / 465,000) x 200,000 = 2.15
    DART 2 cases with either restricted duty/job transfer, or, lost time
    DART = (2 / 465,000) x 200,000 = 0.86

  • SECTION 1: Incident Rates for Canadian Companies

  •
  •
  •
  • The TRC Rate and the Lost Time Modified Duty Case Rate will be calculated by the AGHP as per the following examples

    * As required by Province Workers Compensation Law and/or Occupational Safety & Health Regulations
    ** Rate calculation: rate = # cases divided by employee hours worked multiplied by 200,000
    *** Lost Time or Modified Duty -- the number of individual cases involving a worker who was unable to work at all (due to injury) and the number of cases where the injured worked could work, but at a restricted capacity (i.e. no bending, lifting more than 15 pounds, etc.) or was transferred to another job description (i.e. went from working in the field to working at the shop performing a different task due to the injury).

  • Section 2: Rope Access Program Elements

  • Each “Yes” is worth 5 points, “No” answers are worth zero points, and Section 2 has a possible maximum of 80 points. Some items may require further explanation.
