  • Hello! Thank you for choosing The Lavish Canvas as your tattoo artist!  

    Please ensure that you complete this form within 24 hours of booking your appointment as I won't accept your appointment until I have recieved this form submission. After I have recieved your form, I will message you either through email / text / social media to let you know if I can or cannot create your wanted design. If we are unable to create something to your liking then I will decline your appointment in GlossGenius and you won't be charged the deposit.

    The pricing of your piece will be dependent on the design size, level of detail, placement and colors. A starting price will be provided at the time of booking and final pricing will be determined after the design has been finalized.
