Gift Form
Grace Givers Limits Clients From Giving Gifts to our Staff in Excess of of $50 of value per employee per year. Please note, as a client you are in no way expected to give gifts to any staff at any time, but if you choose to do so, this form must be completed. Occasionally clients may want to give a "bonus" to caregivers, and we ask that any monetary bonus's be approved through the office. We will add this requested amount to your invoice, so that the appropriate taxes etc may be added to the employee's paycheck .
