*All fees must be paid prior to the start of the first lesson

All lessons will be held twice a day.

An email will be sent out one week prior to confirm lesson times

This is the certify that my son/daughter has permission to attend Lifesaving Society Swimming Lessons provided at the Shoal Lake Lakeview Park swimming area. I agree that the participant will comply with any rules, regulations, or directions set forth by the swimming instructor and the Lakeview Park Campground Board of Directors. It is also agreed that an adult will be present during his/her swimming lessons at all times. This parent/guardian or responsible adult accompanying the participant shall be involved in the saftey and well being of the participant before, during and after his/her swimming lessons. The RM of Yellowhead is NOT responsible for the above named participant when they are not participating in their scheduled time for lessons. It is further understood by the undersigned that there will be times at the aformentioned activity when direct supervision of the particiants will not be possible. It is therefore agreed, that while every reasonable precaution will be taken to avoid mishap or accidents during the participants lessons, the RM of Yellowhead is hereby released of all claims or actions during the Shoal Lake Swimming Lessons. I acknowlage that all activities at Lakeview Park Campground and Swimming Area are at our own risk and will not hold the RM of Yellowhead, it's swimming instructors or board members responsible for any mishaps or accidents.
