All Stallions participating in the Louisiana Thoroughbred Breeders Association Program are required to notify the LTBA within thirty (30) days of arrival/entry, or change of ownership. A $65 fee along with this completed form is the responsibility of the Stallion's owner.

In the event an Arrival Form has not been filed with the LTBA with fees paid, a notarized document verifying the Stallion's location and a fine may be imposed.
