Waiver of Liability and Assumption of Risk Agreement and Rental Agreement
I acknowledge that I am voluntarily participating in a test ride or ride of an electric surfboard provided by Ego Depot / Power Rentals.

I understand and acknowledge that participating in activities involving electric surfboards carries inherent risks, including but not limited to the risk of injury, accidents, and property damage. I am aware that electric surfboards can reach high speeds and may involve navigating challenging conditions, including waves and currents.

In consideration of being allowed to test ride and/or ride the electric surfboard, I, on my behalf and on behalf of my heirs, assigns, and legal representatives, hereby assume all risks associated with the activity, including, but not limited to, personal injury, disability, death, and property damage. I release and discharge Ego Depot, its owners, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action, both at law and in equity, arising out of or in connection with my participation in the test ride and/or ride.

I understand that it is my responsibility to follow all safety instructions provided by the staff during the test ride and/or ride. I will adhere to all safety guidelines and use any required safety equipment, including but not limited to life vests and helmets, unless I opt out of helmet use and assume full liability (see below).

I acknowledge that I am responsible for any damage to the electric surfboard or injury to myself or others caused by my negligent or willful actions during the test ride and/or ride.

By signing below, I affirm that I have read and understand the terms of this liability waiver, and I voluntarily and willingly assume all risks associated with the test ride and/or ride of the electric surfboard.

Parent(s) or Court-Appointed Legal Guardian(s) must sign for any participating minor (those under 18 years of age) and agree that they and the minor are subject to all the terms of this document, as set forth above.

By signing below the Parent or Court-Appointed Legal Guardian agrees that they are also subject to all the terms of this document, as set forth above.

Agreement To Bring Required Documents To Appointment:*

By checking here, you are agreeing to bring your valid state issued photo ID and temporary boating certificate (if born on or after Jan. 1st, 1988) or Boater's Safety Card to your reserved ride appointment. A valid state issued photo ID that includes your picture and birthdate is required to rent an electric surfboard. Examples of an ID include a driver's license, passport, or government-issued identification card. Our instructor(s) will take a picture of these documents/items in order to keep them on file as they are required in order to operate the electric surfboard legally. If the rider is under the age of 18, we will require the valid state issued photo ID of the parent/legal guardian as permission granted for their child to operate the electric surfboard. Everyone operating the electric surfboard will need a temporary boating certificate (if born on or after Jan. 1st, 1988) or Boater's Safety Card. Failure to bring these documents/items to your appointment will result in a cancellation of your reserved ride time with no refund.

By checking here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.
