Evergreen Co-op Pre-Authorized Credit Card Form

For added convenience, you can now authorize us to charge your purchases to a credit card of your choice to take advantage of this option.

 

We can not take credit card information in written form.

Please give us a call (403-845-2841) or stop by to have your credit card information stored securely in our system.

Terms & Conditions

 

I authorize the Evergreen Co-operative to charge my  credit card for my future purchases provided or sold by the Evergreen Cooperative, until this authorization is terminated by written notice given by me (us) to the Evergreen Co-operative. I  will notify the Evergreen Co-operative in writing of any changes in the account information immediately. This authorization will continue to apply to me (our) credit card account noted below, even if a new card for that account, with changed expiry date, is issued.

Termination of this authorization to charge amounts to my  credit card will not affect my  liability to pay the Evergreen Co-operative for goods and services provided by the Evergreen Co-operative.

I understand that if a charge appears on my credit card that was not permitted under this authorization or otherwise by me, I must notify my credit card company within 60 days of the date appearing on my statement, in order to have the transaction reversed.
