Adoption fee for Project Freedom Ride Direct Adopt dogs (includes spay/neuter, current vaccinations, registered microchip, negative heartworm test, negative fecal, and health certificate)

(Project Freedom Ride transports to the Pacific Northwest and NE)

Fees are payable via check (if local), PayPal or Venmo and must be paid in advance of PFR transport for our PNW and NE adopters.

Local adoption fee: $225 (includes spay/neuter, current vaccinations, registered microchip, and heartworm test).
Sumitting this application does not mean you are commiting to a dog, nor does it guarantee you will get that dog. It is simply a tool we use to help both you and us identify a good potential match for your family and lifestyle.

I understand that completing this application does not guarantee I will be allowed to adopt a dog, and that the application is a tool to help Levelland Animal Friends determine if this dog and my family are a good fit for each other. I acknowledge that all information provided on this form is true and correct. I also certify that I have never been convicted of animal cruelty or neglect.

I agree to allow a representative from Levelland Animal Friends to contact my vet for a reference and also agree to allowing a home visit or video call prior to adopting this dog. If I am approved to adopt a dog, I agree to pay the above listed adoption fee.

I understand that all dogs adopted from Levelland Animal Friends will be spayed/neutered (puppies adopted locally too young to be altered will be sent with a voucher to be used at the appropriate age), heartworm tested, fecal tested (if adopted through PFR), and vaccinated prior to adoption.
