Waiver and Release of Liability:

I, the undersigned, as the parent/guardian of the participant named above, acknowledge and agree to the following:
1. Assumption of Risk: I understand that participation in activities at The Outer Limits Gaming Lounge Summer Camp involves risks, including but not limited to, physical exertion, interaction with other participants, and potential exposure to various gaming equipment. I voluntarily assume all risks associated with participation.
2. Medical Treatment: In the event of an emergency, I hereby authorize The Outer Limits Gaming Lounge staff to secure medical treatment for the participant named above. I agree to assume responsibility for any costs incurred as a result of such treatment.
3. Release and Waiver: I release and discharge The Outer Limits Gaming Lounge, its officers, directors, employees, agents, and volunteers from any and all claims, liabilities, and expenses arising from the participant's involvement in the summer camp, including personal injury or property damage, whether caused by negligence or otherwise.
4. Photography/Media Release: I grant permission for The Outer Limits Gaming Lounge to take photographs and videos of the participant during camp activities. I understand that these images may be used for promotional purposes, including but not limited to, social media, website, and marketing materials.
5. Behavioral Expectations: I understand that The Outer Limits Gaming Lounge expects all participants to adhere to camp rules and exhibit respectful behavior. The Outer Limits Gaming Lounge reserves the right to dismiss any participant whose behavior is deemed disruptive or harmful to others.
