Program Contract & Provider/Parent Payment Agreement - K3-8th Grade
Please note that if you have unpaid program fees from prior school years, those fees have to be paid in full before you can enroll in programs for the upcoming school year. If you need to check your account balance, login to your account at www.myprocare.com.

Make your program selections from the available programs below. Programs are grouped by grade levels. Seats are limited & are available on a first come basis. If there are no more seats available for a program that you want, you will not be able to register your child for that program. You will have to put your child on the waitlist for any programs that are full.
If you have more than one child, you will have to complete a separate program registration form for each child.
If you qualify for Wisconsin Shares you will need to do the following:
1. Contact MES at 888-947-6583 to add Highland Community School as your childcare provider. Our Provider #9000580989 - Location #001 - EBT Facility #E268468. Your childcare authorization has to be updated by August 30th in order for your child to start programs on their first day of school. If your authorization is not updated by the 30th, you will have to pay the full cost of the first months program fees in order to keep your child's seat in programs.
2. Wisconsin Shares recipients are required to make a $10 deposit (instead of 50% of the first months program fees) in order to secure your child's seat in programs.
If you are a current staff member, you do not have to pay a deposit, but you will have to pay any balance due by the first week of school.
Note: An adjustment will be made to your account for students staggered in after the first day of school.
*There are a limited number of part-time attendance spots in the After Care & K3 Extended Care programs. Part-time spots are limited to 1) Mon, Wed & Fri or 2) Tues & Thurs only. Fees will be prorated based on days selected. All terms apply to part-time attendance as well as full-time. Additional Billing Information: Invoices for the entire month will be posted around the 20th of the month prior & emailed. This is a prepay program. Program payments are due by the 3rd of each month (i.e. May fees will be billed at the end of April & payment is due by May 3rd.
Pay in advance for the year and receive a 4% discount or pay per semester and receive a 2% discount.
