  • GENERAL TRUCKING ACTIVITIES: PROJECT ACCESS REQUIREMENTS

     

  • 1. Working safely is a condition of access. Disciplinary actions are associated with failure to adhere to this or other site-specific requirements. I agree to comply*
  • 2. Drivers must follow and comply with on-site safety signage and regulations. AS well as all federal traffic laws when operating vehicles on the jobsite. I agree to comply*
  • 3. Drivers will never utilize a mobile device/phone while operating a vehicle/equipment on-site. I agree to comply*
  • 4. Drivers may only exit the vehicle for loading and unloading. Driver must stay within the vicinity of truck. If leaving the loading area, drivers must be escorted by NPCL. I agree to comply*
  • 5. If exiting vehicle, driver MUST wear required on-site PPE: hard hat, safety glasses, safety gloves, steel/composite toe boots and long pants. I agree to comply*
  • 6. Prior to loading or unloading, verify load hasn't shifted during transport. During loading or unloading, no workers should be standing in area below load where shifted materials may fall. I agree to comply*
  • 7. When loading/unloading to or from a flatbed, driver must use a trucker ladder for access. No climbing/jumping is permitted. I agree to comply*
  • 8. Drivers will park only in designated areas specified by your point of contact or NPCL. I agree to comply*
  • 9. Drivers will be aware and stay clear of forklifts and other heavy equipment on-site. I agree to comply*
  • 10. All incidents including near misses, injuries/first aid, equipment/property damage, or environmental releases must be reported to NPCL immediately, regardless of severity. I agree to comply*
  • 11. In the event of an emergency, you will hear 3 whistles or horns, indicating that you are to stop work, and assemble at the primary emergency muster point (roundabolut). I agree to comply*

    By signing this form, I acknowledge that I have read, understand, and will comply with the policies and procedures as set forth in this orientation form. I acknowledge that this orientation is specific to delivery driver/vendor activities only. A full HSE orientation will be required for any scope of service outside of delivery activities. This form will be retained on file at the project worksite location.
