West Park High School Official Transcript Request Form

An official transcript is one that is received directly from the issuing institution. It includes the school seal and signature of the registrar or designee and is placed into a sealed envelope.
CURRENT STUDENTS/GRADUATES

All transcripts must be requested by submitting the Transcript Request form.

All transcript requests are processed in the order in which they are received, regardless of the method by which they are requested. Please allow up to 7-14 days for processing. Official transcripts cannot be emailed. They can be mailed to the requestor or picked up in person. Please pick up transcripts at the WPHS front desk from 1:00 p.m. to 3:00 p.m., Monday through Friday.

Please note that the transcript will only be sent request by the student and after payment of the transcript fee(s). WPHS does not issue partial records. Processing an official transcript request usually takes 7-14 business days processing time. Before processing the transcript, the student must validate that all the information in their Student Account Information is accurate. WPHS is not responsible for transcripts sent to an inaccurate recipient address provided by the student.

When applicable, official courses-by-course evaluation from a NACES member credential evaluation service is required for all international post-secondary (college or university) coursework.
