COLLECTION, USE AND DISCLOSURE OF PERSONAL INFORMATION

RDRHDA collects, uses and discloses your personal information only for the following purposes: establishing and maintaining communications regarding the association; meeting legal, security, regulatory and processing requirements; compiling statistics for the analysis of association metrics, and organizing and hosting competitions, dance-outs, workshops, fundraisers, medal testing and other functions relating to Highland Dancing.

WAIVER

I am aware that participating in any Event hosted or organized by the Red Deer Region Highland Dancing Association (the “Event”) involves the potential for risks, dangers and hazards, including but not limited to:

(a) injury to myself / my child / my guests / my additional family members as a result of the physical nature of dance competition and performance; and

(b) the taking and/or release of unauthorized photographs, video and/or other recordings of myself and/or my child / my guests / my additional family members, notwithstanding efforts to enforce Event policies.

In consideration of my / my child’s / my guest's / my additional family members participation in the Event and for other good and valuable consideration, the receipt and sufficiency of which is acknowledged, I hereby WAIVE ANY AND ALL CLAIMS against Red Deer Region Highland Dancing Association, Alberta Highland Dancing Association, ScotDance Canada, its directors, agents, employees, executive members, volunteers and organizers of the Event (the “Releasees”), which I now have, or hereafter may have against the Releasees. In particular, but without limiting the foregoing, I agree to release the Releasees from ANY AND ALL LIABILITY for any loss, damage, injury, or expense I / my child / my guests / my additional family members may incur as a result of my / my child’s / my guests / my additional family members participation in or attendance at the Event. I agree to indemnify and hold harmless the Releasees from ANY LOSS, DAMAGE, INJURY OR EXPENSE, including legal costs, that the Releasees may incur due to my / my child’s / my guests / my additional family members participation in or attendance at the Event, whether caused by the negligence of the Releasees or otherwise, and whether caused to my child or otherwise. I understand that the Releasees will photograph and otherwise record the Event, its participants and attendees. I consent to and authorize such recording. I further consent to and authorize the Releasees to publish my / my child’s/ my guests / my additional family members image, likeness and information in any medium, including but not limited to photographs, videos, DVDs, and web-casts. I understand that this publication will take place in the public domain, including in printed materials, public announcements, and on the internet.

I specifically consent to the public release of the following information: 1. Name; 2. City, Province, Country of Residence; 3. Tartan; 4. Age; 5. Results.

I acknowledge that by signing and submitting this entry, electronically or otherwise, that I have read this agreement carefully and fully understand it, and agree to all of the above terms and conditions.

MEMBERSHIP TYPE

The RDRHDA offers 4 levels of membership:

FAMILY - Parents/Guardians and their dancers *UNDER the age of 16 (dancers 16+ MUST purchase an Independent Dancer Membership)*. This level includes a fund-raising account through the association, dancing privileges at RDRHDA competitions, medal testing venue costs, association hosted workshops, pipe practice, eligibility for annual bursaries, elibility for SDCCS Provincial Representative sponsorship, a vote at RDRHDA meetings, and right to hold executive positions within the association. Pricing consists of a base fee, plus an additional per dancer fee.

INDEPENDENT DANCER - Independent dancers *MUST BE OVER 16* years of age (taken on December 31st). *Independent dancers aged 16 & 17 MUST have a parent or guardian fill out the Primary Member / Guardian section of this form. Parents / guardians ensure you have read and agreed to all conditions and waivers included in this form.* This level includes a fund-raising account through the association, dancing privileges at RDRHDA competitions, medal testing venue costs, association hosted workshops, pipe practice, eligibility for annual bursaries, elibility for SDCCS Provincial Representative sponsorship, a vote at RDRHDA meetings and right to hold executive positions within the association.

ASSOCIATE - Associate members *MUST BE OVER 16* years of age (taken on December 31st). *Associate members aged 16 & 17 MUST have a parent or guardian fill out the Primary Member / Guardian section of this form. Parents / guardians ensure you have read and agreed to all conditions and waivers included in this form.* This level of membership DOES NOT include fund raising priviledges! Included are a vote at RDRHDA meetings, and the right to hold executive positions within the association. No dance related privileges are included with this membership type. It is intended for teachers, and parents of Independant Dancers 16+ years who hold their own membership.

LIFETIME MEMBER - Any member, for valuable services rendered to the local association may be nominated for Honorary Life Membership in the local. Such nomination shall be made in writing to the Executive of the local and shall clearly state the name of the nominee, the reasons for the nomination, and shall be signed by 10 ordinary, associate, or life members in good standing. Upon approval by the local such nomination shall be registered with the Provincial Association. Upon registration the member shall admitted to Honorary Life Membership and shall be presented with a life membership card by the Provincial Association. Life Members shall be entitled to all the privileges of ordinary membership but shall not be required to pay any further fees, dues, or subscriptions.
