Registration Details:

Please fill out this form to register your daughter for the basketball fundamentals and skills training program offered by Hoopers Academy. This program is designed to improve your daughter’s basketball handling, passing, shooting, footwork, and defensive skills, as well as her confidence and teamwork.

Hoopers Academy runs each clinic for 5-8 weeks. Hoopers Academy will provide each participant with one t-shirt. Once received, ALL Hoopers must wear their Hoopers Academy t-shirt to practice each week to encourage equity and unity. (ALL SALES ARE FINAL AND NON-REFUNDABLE AFTER 24 HOURS)

Waiver & Consent (Hoopers Academy @ TPS)

I, the parent/guardian of the participant, hereby give my permission for my child to participate in the basketball skills program offered by Hoopers Academy at Tuxedo Park School. To the best of my knowledge, my child is physically fit and able to participate in these activities. I understand that there are inherent risks involved in any physical activity, and I agree to assume all responsibility for any injuries or damages that may occur to my daughter or myself as a result of her participation. I also agree to waive, release, and hold harmless Hoopers Academy, its staff, volunteers, and affiliates from any claim arising out of any injury to my child(ren), whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance. I understand and assume all risks and hazards associated with participation in these activities, including the transportation to and from the activities, and do hereby waive, release, absolve, indemnify, and agree to hold harmless to the extent permitted by law, the Tuxedo Park School, The Tuxedo Club, Village of Tuxedo Park, organizers, sponsors, participants, and all persons and companies providing such activities and transporting my child(ren) to or from activities, from any claim arising out of any injury to my child(ren), whether the result of negligence or for any other cause, except to the extent and in the amount covered by accident or liability insurance. In the case of a medical emergency, I authorize the Hoopers Academy representatives to seek medical treatment for my child at the nearest medical facility and they may call paramedics and discharge my child to an ambulance if I am not able to authorize it at the time of the medical emergency. I also agree to waive, release, and hold harmless Hoopers Academy, its staff, volunteers, and affiliates from any and all claims, liabilities, or damages arising from or related to my daughter's participation in the program. I also give my consent for Hoopers Academy to use any photographs, videos, or testimonials of my daughter taken during the program for promotional or educational purposes, without any compensation or prior notice. I understand that I can withdraw my consent at any time by contacting Hoopers Academy. ALL SALES ARE FINAL AND NON-REFUNDABLE. There are NO make-up sessions offered at this time.
