A Pet Care Agreement is a contract between two parties for pet care services: the pet owner and the pet care provider. When a pet owner requires someone to care for his or her pet (dog or cat, for example), this agreement clearly states your pet's signalments, habits, and medical requirements as well as what is expected of the pet care provider.

Representation of Ownership: The Owner hereby represents that they are the pet's legal owner (details of the pet are provided below), and they have full authority to enter into this agreement.

Authorization for Pet Care: The Owner hereby authorizes the Provider, MSPetsAdventures, to care for their pet.

The Owner agrees to the following terms and conditions:

Ownership Confirmation: The Owner confirms that they are the pet's legal owner.

Vaccination: The Owner confirms that their pet is up-to-date on all vaccinations per Nevada state law. *Rabies required only* *Other vaccines may be required for different services*

Tick/Flea Control: The Owner confirms that their pet is treated for tick and flea control. *Required for hiking services only*

Health Status: The Owner confirms that their pet is in good health. *Required for certain services, may need proof of annual wellness examination by a veterinarian*

Behavior: The Owner confirms that their pet is friendly enough to be in public and has never bitten another person. *Required for certain services, may need behavioral training if any incidents arise*
Terms and Conditions

1) Payment must be made upfront or by the invoice due date and may include extra fees or charges for additional services, such as pet, travel, spicy, or medication fees. All transactions are done through a third-party vendor, Quickbooks; however, we accept cash, Zelle, and Cash App. By signing this contract, the client acknowledges their understanding and agrees to pay the stipulated fees. This signed contract shall also serve as ongoing authorization for MSPetsAdventures to accept reservations via phone or email and access the premises without further written contracts or authorizations.
2) MSPetsAdventures is authorized to hike/walk the pet as this contract outlines and services are required.
3) MSPetsAdventures has a no-off-leash policy for liability reasons. Your dog will never be let off leash unless we use personal secured property. (Written approval for dog parks and other approved locations).
4) MSPetsAdventures is authorized, in the best interest of both parties, regarding inclement weather; hikes/walks will not occur if the temperature is below 20F or above 95F or if there are high winds or heavy rain/flooding. In inclement weather, the pet will be given a potty break (brief walk if possible), and the rest of the time will be spent playing indoors.
5) In the event of personal emergencies or illnesses, MSPetsAdventures will notify the client promptly, and fees for missed services will be reimbursed.
6) MSPetsAdventures reserves the right to terminate this contract at any time before or during this term.
7) Cancellations must be made at least 24 hours in advance. Those with less than 24 hours notice may incur charges at the total rate, while cancellations made more than 24 hours in advance qualify for a reschedule and possible credit.

Medical Treatment Agreement

Emergency Contact
MSPetsAdventures is authorized to obtain any necessary medical care within reason up to an amount of     , and is released from all liabilities concerning transportation, treatment, and related expenses. MSPetsAdventures can also approve medical and/or emergency treatment (excluding euthanasia) as a veterinarian recommends. The client agrees to reimburse MSPetsAdventures for the above expense incurred while attending to this need.

Release of Liability

I waive and release MSPetsAdventures and their employees from any

liabilities for any reason, including my actions (the owner), my pet(s), or any

other person accompanying me (the owner) or holding a key to my home. MSPetsAdventures agrees to provide all services in a kind, humane, reliable, and trustworthy manner. The client agrees to notify MSPetsAdventures of any concerns within 24 hours of their return. In case of an emergency, inclement weather, or a natural disaster, I authorize MSPetsAdventure to use their reasonable judgment for the care and well-being of my pet(s) and/or house.

I acknowledge as the owner that I am responsible for medical expenses and damages resulting from an injury to MSPetsAdventures or other persons or animals caused by my pet(s). MSPetsAdventures reserves the right to refuse service to any client at any time. I attest that all of the above information is true to my knowledge. If anything changes from what is listed, I will inform MSPetsAdventures before the next service is scheduled. This signed document authorizes MSPetsAdventures to enter the above-listed address to perform the necessary service. I authorize this contract to be valid approval for services to permit MSPetsAdventures to accept all future telephone, email, mail, and online reservations and enter my home without additional signed contracts or written authorizations.
