Baptism Certificate Request Form

This certificate will include notations regarding Confirmation, Marriage and Annulments, along with Baptisms.
Use this form if your baptism occurred, in Fond du Lac, since July 1, 2000 at:
- Sacred Heart Church site,
- St. Joseph Church site,
- St. Louis Church site,
- St. Mary Church site,
- St. Patrick Church site,
- St. Peter Church site, and
- Holy Family
OR since July 1, 2020 for
- Our Risen Savior, and
- Presentation.
(ORS and PBVM joined Holy Family in 2020; so we will not have any records from those sites prior to July 2020. We will have records from the other sites starting in July 2000.)

If you were baptized at the former parishes before July 2000, please contact the archives at the Archdiocese of Milwaukee using their Sacramental Request form.

If the baptism took place at a parish other than the ones listed above please contact that church for the certificate.
