Live Stream Request Form

PLEASE READ THE FOLLOWING BEFORE CONTINUING!

Please be advised that this request form is not a confirmation of live stream services. Each request will be evaluated and considered in the context of other events being held concurrently at the College. While we will do our best to fulfill each request, live streaming requests received within 30 days of the event may not be filled.

We can only accept requests for live stream support for events that have already been submitted to Theater and Event Support Services for review. You'll need an EMS Reservation ID Number to complete this form. If you do not know yours, you can reach out to your Event Coordinator to receive it. If you have multiple event dates that have different Reservation IDs, you'll need to complete this form for each Reservation ID separately.

The service being provided is one-way live streaming only. This DOES NOT include video conferencing through Zoom, Skype, Webex, or other platforms meant for two-way interactions with audio and video communication. What we're providing is viewing access for a remote audience.

Do you have access to an account you'd like to host this stream? (We will not need password access to this account, simply a stream key and URL from the livestream configuration page which we can assist you in providing.)

Would you like the audience to be able to text chat during the stream? If so, you'll need to have a member of your group present to manage chat moderation and Q&A.

We require a representative from your group to be present to manage audience support requests and communication. We DO NOT provide staff to handle tech support requests from the audience.

We will provide a form where this can be completed later in the process, after your stream is confirmed (without this we cannot guarantee that speakers will be identified and labeled correctly while onscreen):
