Kiowa's House Husky Rescue Application

We appreciate your interest in Adopting or Fostering with Kiowa's House Husky Rescue. 

You agree to assume full financial, physical, and mental responsibility for the care of the dog. You also agree to work through any adjustment phases that may arise. Should you be unable to complete the application in one session, you may save your progress and return to it at your convenience.

*Adopters must be 18 or older with valid state-issued photo ID

I understand that I may not surrender the animal or take the animal to any animal Shelter. If I am unable to keep the Pet, I agree to contact Nicole VanKampen immediately.

All of the above information I have given is true and complete. I agree to follow the rules and policies. I will not hold Nicole VanKampen responsible for any damage, injury, or harm caused directly or indirectly to any person or property by the Pet. I have read the disclaimer and agree to these terms and conditions.

1. ADOPTION PROCESS: I agree to pay the Adoption Donation to help Nicole VanKampen with expenses for food, sheltering, spaying or neutering, vaccinations, and veterinary care.

2.YOU MUST PROVIDE THE PET WITH PROPER VETERINARY CARE: I agree to provide the Pet with the necessary vaccinations as advised by the Pet’s veterinarian. I agree to obtain immediate veterinary care should the Pet become sick or injured, and to take full financial responsibility for any veterinary expenses. Nicole VanKampen is not responsible for veterinary expenses once the animal is adopted.

3.THE PET MUST BE TREATED AS AN INDOOR PET: I agree to treat the Pet as an indoor pet and household companion, not as an outdoor pet. The means that the Pet must be kept inside the home in a safe place at all times unless supervised outside.

4.THE PET MUST BE PROVIDED WITH HUMANE TREATMENT AT ALL TIMES: This means providing the Pet with proper food, fresh water, proper shelter and exercise, and allowing a reasonable amount of time for the Pet to adjust to new surroundings.

5.Nicole VanKampen MAKES NO REPRESENTATIONS OR GUARANTEES AS TO THE TEMPERAMENT OF THE PET: I understand that there is no guarantee of any kind as to the health, temperament, or training of the Pet. I understand that Nicole VanKampen makes no representation or guarantee of any kind, either explicitly or implicitly, as to the health, temperamental, mental disposition, or training of the Pet.

6. I understand that if my dog is not already spayed/neutered that I will move to a foster to adopt agreement until the spay/neuter is completed. Kiowa's House Husky Rescue will cover costs associated with the spay/neuter. 

I understand that I may not take the animal to any Animal Shelter, Humane Society, or give the animal away on my own unless prearranged through an adopNon contract addendum with Nicole VanKampen.

1.RELEASE OF LIABILITY: The Adopter/foster agrees never to bring a claim or suit against Nicole VanKampen. The Adopter/foster releases Nicole VanKampen from all liability arising from the adoption or behavior or actions of the Pet.

I, the Adopter/foster, agree that I am legally competent to enter into this agreement. I am the only household decision maker regarding pet adoption, and this agreement is binding.
