Welcome to The Well Academy's Enrollment form. We are excited you have chosen to homeschool through us and we want to ensure that your enrollment process is as simple as possible.

On the next page you will see our Statement of Cooperation. Please read over it carefully as it is our agreement between The Well Academy and your family.

Statement of Cooperation

Please carefully read our statement of cooperation below. Your eSignature and submission of this form constitutes your agreement with the Statement of Cooperation.
1. We are implementing an appropriate education program with associated resources to be used during the year for each of our children. (If this is your first time enrolling with The Well Academy (TWA) you must submit a copy of your initial study plan.)
2. We will maintain a study plan/journal for each child. We agree to make this plan/journal available to the school administrator or designated representative upon request.
3. We will maintain an attendance record for each child showing the days of attendance per calendar year. Furthermore, our children will be directed during the day by ourselves or a designated adult guardian.
4. We will, as the designated Church School Teacher for our children, prepare a Progress Report or Transcript Report, as applicable, for both MID-YEAR and END-OF-YEAR record keeping. We agree to make our reports available to the school administrator or designated representative upon request.
5. We will, as the designated Records Custodian, maintain a record file for each child. This file will contain enrollment records, study plan/journal, attendance data, reports, and curriculum materials completed during the school year.
6. We agree to contact and support the TWA office if we transfer our children to another school that requests submission of records by The Well Academy office. Well Academy will support the records transfer process as requested with the necessary input provided by you, the Record Custodian. An additional service fee may apply.
7. We realize that to continue our enrollment with TWA we must re-enroll each year in JUNE-JULY. After August 20th we realize that we will be withdrawn from TWA and could become subject to truancy laws if we fail to enroll elsewhere. We will report any change of address or contact information.
8. For new enrollments, $100 of our registration fee is non-refundable. After 30 days from the date of our registration on this form, no refund will be given.
9. We release the TWA school administrators, coordinators, and other support staff from all responsibility or liability for the education and the social and physical well being of our children. We agree to assume all risk for the wellbeing of our children at any functions or activities sponsored by members of TWA.
10. We acknowledge and confess that Jesus Christ is Lord in our hearts and home. Our children will study the Bible as part of our home education program and we purpose to conduct our lives in accordance with these teachings.
11. We agree to comply with the guidelines set forth in this Statement of Cooperation.
