Tumbling Waiver and Release of Liability
I, [Participant Completing Registration], hereby agree to the following terms and conditions in consideration for being permitted by Powerhouse Athletix to participate in tumbling activities:
1. **Assumption of Risks:** I understand and acknowledge that tumbling activities involve inherent risks, including but not limited to falls, collisions, and injuries. I voluntarily assume all risks associated with my participation in tumbling activities at Powerhouse Athletix.
2. **Release of Liability:** I hereby release, waive, discharge, and covenant not to sue Powerhouse Athletix, its owners, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of any damages, injuries, or losses sustained by me while participating in tumbling activities at Powerhouse Athletix.
3. **Indemnification:** I agree to indemnify and hold harmless Powerhouse Athletix, its owners, officers, employees, agents, and volunteers from any and all liabilities, claims, demands, actions, or causes of action, including reasonable attorney fees and costs, arising out of or related to my participation in tumbling activities at Powerhouse Athletix.
4. **Medical Treatment Authorization:** In the event of any injury or medical emergency arising during my participation in tumbling activities at Powerhouse Athletix, I authorize Powerhouse Athletix and its representatives to obtain medical treatment for me, including transportation to a medical facility, if deemed necessary. I understand that I am solely responsible for any costs associated with such medical treatment.
5. **Fitness to Participate:** I affirm that I am physically fit and sufficiently trained to participate in tumbling activities at Powerhouse Athletix. I agree to immediately notify Powerhouse Athletix of any physical or medical conditions that may affect my ability to safely participate in such activities.
6. **Photography and Publicity:** I consent to the use of any photographs, videos, or other media taken of me during my participation in tumbling activities at Powerhouse Athletix for promotional and publicity purposes without compensation.
7. **Governing Law:** This waiver and release shall be governed by and construed in accordance with the laws of Virginia.
I have read this waiver and release of liability, fully understand its terms, and voluntarily agree to be bound by them. I acknowledge that I am signing this agreement freely and voluntarily, and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law.
