Occasionally, an emergency arises when it is necessary for a JosiahKids representative to contact parents when their children are in session with us. Every effort will be made to notify the parents or someone designated by them if a child should become very ill or be involved in an accident. If this cannot be done, the policy of JosiahKids is to have the child transported to the nearest emergency hospital. This action will be taken in all such cases unless instructions to the contrary are provided by the parents.

By completing and submitting this application, (we), the undersigned, parents/guardians of a minor(s), do hereby authorize JosiahKids as agent for the undersigned to consent to any x-ray examination, anesthetic, medical or surgical diagnosis or treatment, and hospital care which is to be rendered under the general or special supervision of any physician or surgeon licensed under the provisions of the Georgia Composite Medical Board Act on the medical staff of a hospital, whether such examination, diagnosis or treatment is rendered at the office of said physician or at such a hospital.

It is understood that this authorization is given in advance of any specific examination, diagnosis, treatment or hospital care being required and is given to provide authority and power on the part of our above-named agent to give specific consent to any and all such examinations, diagnoses, treatment or hospital care which the aforementioned physician in the exercise of his/her judgment may deem advisable.

This authorization is given pursuant to the provisions of Section 25.8 of the Civil Code of Georgia and will cover attendance at all authorized functions.

COST & TUITION

Tuition & Cost: The cost of our program for the 2024/2025 School Year is $350/per student. Tuition paid to JosiahKids is applied toward our nonprofit's operating budget and is due during enrollment.

IMPORTANT NOTE: The IRS does not classify tuition as a tax-deductible expense. Tuition is not the same as a donation.

Payment Plan: A minimum, non-refundable deposit of $100.00 must be paid upfront if unable to pay the tuition in full during registration. A five (5) month payment plan of the balance is available for families that need one. The $100 deposit is non-refundable and will be applied towards the student's tuition. 

JosiahKids Dress Code: To meet our dress code guideline, every JosiahKids student is aslo required to be in a black JosiahKids shirt or hoodie each time we are in session. This year, when paying your tuition, you will be given the opportunity to place your merchandise order in the quantity and sizes you need. Remember our dress code guideline is a security measure, and therefore not optional.

Waiver & Behavioral Agreement: A waiver and behavioral agreement must be signed by each student AND parent each school year for an application to be reviewed for acceptance.

2. Sign the annual waiver (only one waiver is needed per family each year)

3. Sign the behavioral agreement (parent AND student must read and sign)

No students will be confirmed for enrollment without completing each of these steps. Students will be notified of their acceptance on August 15th, 2024.
