Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Shelley's Dance Company Ltd. during the selected camp. In exchange for the acceptance of said child’s candidacy by Shelley's Dance Company Ltd., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Shelley's Dance Company Ltd. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Shelley's Dance Company Ltd. including all teachers and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all dance activities. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

FOIP Release

Shelley's Dance Company may utilize my child's photographic image and/or name in its public relations and communication materials created between a period of a year from the date consent is signed. I realize that I may withdraw my consent in writing at any time by contacting Shelley's Dance Company. I understand that the photograph(s) and/or name may be used in a publication, print advertisement, direct-mail piece, electronic media (e.g., social media applications, video, CD-ROM, internet, World Wide Web, etc.) or other form of communication. In giving my consent, I hereby release and hold harmless Shelley's Dance Company and their agents, employees, officials, representatives and contractors from any and all responsibility or liability for damage of any kind suffered in any matter whatsoever. I hereby relinquish any and all personal or proprietary rights I may have in connection with such use. I understand that I will receive no compensation should any photograph of me be used.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
