Cheer Camp Waiver of Consent and Liability Release
**Acknowledgment of Risk:**
I, the undersigned, understand that participation in the Cheer Camp involves physical activity that may result in injury. I acknowledge that cheerleading and related activities carry inherent risks, including but not limited to sprains, strains, fractures, and other physical injuries.
**Consent to Participate:**
I hereby give my consent for the participant named above to attend and participate in the Cheer Camp. I acknowledge that participation is voluntary and that I have the right to withdraw the participant at any time.
**Medical Authorization:**
In the event of a medical emergency, I authorize the staff of the Cheer Camp to obtain necessary medical treatment for the participant. I understand that every effort will be made to contact the emergency contact listed above. I agree to bear any costs associated with such treatment.
**Liability Release:**
In consideration of being allowed to participate in the Cheer Camp, I hereby release and hold harmless the camp organizers, staff, volunteers, North Carolina A&T State University, and any affiliated organizations from any and all liability, claims, or causes of action arising out of or related to any injury, accident, or illness that may occur during the participant’s involvement in the Cheer Camp.
**Photo/Video Release:**
I grant permission for the Cheer Camp to use photographs and/or video recordings of the participant for promotional purposes, including social media, websites, and printed materials.
**Acknowledgment of Understanding:**
I have read this waiver of consent and liability release, fully understand its terms, and understand that I am giving up substantial rights, including the right to sue. I acknowledge that I am signing this agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
