Hair Salon New Client Consent Form
Kindly read the following information below and sign in to acknowledge it
I confirm that the salon and the hairstylist will not be responsible or liable if the result of the service is not as expected as it should be and I understand it may take more than one session to reach my goal.
I understand that with colour corrections the result may take several attempts and if at any point my stylist feels like the hair cannot take anymore that I am okay to stopping at this process.
I understand that wearing jewellery, hoods, high neck tops may get damaged with chemicals used in the salon.
I am allowing the salon and the hairstylist to apply necessary chemicals as part of the service in my hair treatment.
I give permission and waive liability to my hairstylist incase of any negative reactions caused by chemical services reactions I or my child is receiving.
I have discussed with my stylist the history of what has been used on my hair before the colour service to make sure we know what to expect when colouring the hair.
I consent the salon to take photographs of the provided service and sharing the photos on social media.
