As you have filled out the form completely you are one step closer to being able to get the tattoo you have been dreaming about. I will get to your request as soon as possible. To request another design one will need to fill out a new request form. We will book accordly to your schedule to make the best times that make work for you. As I look over the request form understand that you would need to be of 18 years or older to receive the tattoo. Being accompanied by a parent would be suggested if needed. Ensure identification is ready to go in the form of a drivers license or a passport. Eating a small meal and stay a bit hydrated is key before getting tattooed. One person may be allowed during tattoo session. Sessions can be broken up into multiple sessions is passing the 3 hour mark if needed.
