I, undersigned, agree with the following statements:
I UNDERSTAND THAT THERE ARE RISKS ASSOCIATED WITH RECEIVING PERMANENT JEWELRY
I UNDERSTAND THAT IF I HAVE A HEART CONDITION OR SENSITIVE SKIN, PERMANENT JEWELRY IS NOT RECOMMENDED.
I UNDERSTAND THAT "PERMANENT" DOES NOT MEAN IT WILL LAST A LIFETIME. PERMANENT JEWELRY HAS A LIFE EXPECTANCY OF MONTHS-YEARS DEPENDING ON THE WEARER'S LIFESTYLE. IT IS STILL FINE JEWELRY AND NOT INDESTRUCTIBLE.
I UNDERSTAND THAT ANYONE UNDER THE AGE OF 16 MUST HAVE BE ACCOMPANIED BY AN ADULT.
I UNDERSTAND THAT IF PERMANENT JEWELRY NEED TO BE REMOVED, IT CAN BE DONE BY USING NAIL CLIPPERS OR SCISSORS TO CUT AT THE JUMP RING. A SMALL RE-WELD FEE MAY BE REQUIRED ($10+) OR A CLASP MAY BE PURCHASED.
I UNDERSTAND THAT MOST JEWELRY HAS TO POTENTIAL TO TARNISH OR CAUSE DISCOLORATION OF THE SKIN BASED ON A VARIETY OF FACTORS, SUCH AS THE WEARER'S LIFESTYLE OR SKIN PH.
I UNDERSTAND THAT ALL SALES ARE FINAL ONCE THE CHAIN HAS BEEN CUT. WE CANNOT OFFER REPLACEMENTS, REFUNDS OR REPLACE LOST CHAINS DUE TO THE PERSONAL NATURE OF THE WEARER'S LIFESTYLE, JOB OR OTHER FACTORS OUT OF OUR CONTROL.

I UNDERSTAND THE WARRANTY BELOW:

BREAKAGE: If permanent jewelry breaks AT THE WELD SITE within 30 days, we will re-weld by appointment at no charge. After 30 days, a small fee of $10+ will be required to re-weld.

LOST JEWELRY: If permanent jewelry is lost within 30 days of the appointment, a 20% discount may be applied toward the purchase of a replacement.
