    I acknowledge and understand that meaningful participation in the project includes:

    • Demonstrate/document active participation as determined by the project completion requirements (length of participation);
    • Be intellectually engaged in planning and executing the project;
    • Implement the project’s intervention (the changes designed to improve care);
    • Review data in keeping with the project’s measurement plan; and
    • Collaborate activity by attending virtual collaborative sessions and participate during current certification period or MOC cycle.

    I also understand that this is an interest form only, and does not guarantee that I will be selected as a participant. Selected participants will receive a separate acceptance email in July 2024.

  • 13. I acknowledge that if I am accepted into this project, I need to provide documentation of previous QI experience and understanding of QI concepts. I also understand that if I have not completed QI training or participated in a QI project previously that I will be expected to complete a QI basics training and submit documentation of completion prior to the conclusion of this project.

    Additionally, resources on QI basics will be provided to you that can be completed in April before the project starts.
