I acknowledge that there are risks involved in getting a tattoo, both those that are known and those that may not be fully understood. These risks include, but are not limited to, infection, scarring, challenges in detecting melanoma, and allergic reactions to tattoo pigment, latex gloves, or soap. Despite being aware of these potential risks, I am choosing to proceed with the tattoo procedure. I accept full responsibility for any consequences that may arise from this decision.
I hereby waive and release Christian Rios CEO of Ink Hero from any and all liability, to the fullest extent permitted by law. This includes, but is not limited to, any claims or causes of action that I, my estate, heirs, executors, or assigns may have for personal injury or any other damages, whether direct or consequential, arising from the procedure and application of my tattoo. This release encompasses negligence or fault on the part of Christian Rios or any other party involved.
Christian has provided ample opportunity for me to inquire about the procedure and application of my tattoo, and any questions I had have been thoroughly addressed to my complete satisfaction.
Furthermore, Christian has provided me with detailed instructions for the proper care of my tattoo during the healing process, which I fully understand and intend to adhere to. I acknowledge the potential risk of infection if these instructions are not followed diligently.
In the event that touch-up work on the tattoo becomes necessary due to my own negligence, I agree that I will be responsible for covering the expenses associated with such work.
I am not under the influence of alcohol or drugs
I do not have acne, freckles, moles, or sunburn in the area to be tattooed that might be agitated by the tattoo process (healing excluded)
I have looked over my design, checked the spelling if applicable, and give my full consent to the application of my tattoo
I take proper care of my tattoo, and I have been advised of the signs and symptoms of infection that indicate a need toseek medical care.
I acknowledge that variations in color and design may exist between any tattoos as selected by me and as ultimately applied to my bodv.
I acknowledae that tattooing is a permanent change to my appearance and that no representations nave been made to me as to the abilitv to later change. alter or remove my tattoo.
I acknowledge that the obtaining of my tattoo is my choice alone and I consent to the application of the tattoo and actions or conduct of the associates agents
I acknowledge that tattoo inks, dyes and pigments have not been approved by the federal Food and Drug Administration and the health consequences of using these products are unknown
I acknowledge that there is a chance I might feel lightheaded, dizzy during or after being tattooed. I agree to immediately notify the practitioner in the event I feel lightheaded, dizzy and/or faint before, during or after the procedure
I agree to follow all instructions concerning the care of my tattoo, and that any touch-ups needed because of my own negligence will be done at my own expense
I certify that all information provided in this form is truthful, complete, and accurate. I absolve this organization of any responsibility in the event of accident, illness, or injury.
I have thoroughly reviewed the policies outlined on Christian Rios's Instagram and agree to strictly adhere to them. I acknowledge the possibility of being asked to provide before and after photos, and I grant Ink Hero permission to utilize these images for marketing or promotional purposes. By signing below, I confirm that I have had the opportunity to ask any questions regarding the procedure(s) to be performed by Christian Rios, and all inquiries have been addressed to my satisfaction.
I have carefully read and comprehended each paragraph above. I understand that this constitutes a contractual agreement and that no warranties or guarantees have been provided regarding the outcomes or consequences of the aforementioned procedure(s). Furthermore, I affirm that I was in a sound state of mind and capable of making independent decisions at the time of signing this consent.
Finally, I acknowledge that the tattoo will be administered in a residential setting, as Christian Rios operates from his home. I am aware that he is a developing artist working to build his portfolio, and I understand that minor errors may occur. Nonetheless, I recognize this as a professional environment and will conduct myself accordingly.
