My child must comply with the DIOCESE OF NASHVILLE'S OFFICE OF FAITH FORMATION rules and procedures. By granting this permission, I agree on behalf of myself, my child named herein, or our heirs, successors, and assigns, to hold harmless and defend THE OFFICE OF FAITH FORMATION, its officers, directors, employees, and agents, and the Diocese of Nashville, its employees and agents, chaperones, or representatives associated with the event, from any claim arising from or in connection with my child attending the event or in connection with any illness or injury (including death) or cost of medical treatment in connection therewith, and I agree to compensate the parish, its officers, directors and agents, and the Diocese of Nashville, it's employees and agents and chaperones, or representatives associated with the event for reasonable attorney's fees and expenses which may incur for any action brought against them as a result of such injury or damage, unless such claim arises from the negligence of the parish/diocese.

Payment Information

The cost to attend a Search for Christian Maturity Retreat is $195 and includes all meals and lodging for the duration of the retreat.
Payment: If I submit this registration form, but do not submit payment, I am not fully registered, and my spot will not be guaranteed. Payment for my registration fee must be received by the Diocese of Nashville's Office of Faith Formation for Youth and Young Adult Ministry at least 10 business days in advance of the retreat start date.*

Reschedule: If I wish to switch my registration to a different retreat weekend, I must request to be rescheduled by contacting the Diocese of Nashville's Office of Faith Formation for Youth and Young Adult Ministry at least 10 business days in advance of the retreat start date. After this date, the registration fee will not be transferable.*
