    Ribbons: To 5th Place

    Dogs: All dogs MUST be on a leash at all times. Violation fine: $50 per incident. 

    Stalls: $100 for the weekend or $35 per night. No bedding provided. Upon moving out, please pick your stalls out. Stalls open Thursday at 12noon and must vacate by 3 pm Sunday. If stall is needed Sunday night, please make arrangements for a dry stall at Maple Crest, $25/night 

    Tack Stalls: $45 for the weekend or  $15 per night.  

    Stallions: Please note if stabling, accommodations can be made.

    Non-competing Horses: Horses attending, but not competing, are welcome if stalls are available. Competing horses will be given priority. Please check with the Show Secretary. Stall fees are the same as competing horses. Waiver & Release required. 

    Camping: $25/night per campsite. Shower and toilet facilities are available.  

    Entry Form: Please use an official separate entry form (with all contact info given) for every horse/driver combination.

    The undersigned has agreed to participate in any capacity (including as a rider, driver, handler, lessee, owner, agent, coach, official, trainer, or volunteer) in one or more MAPLE CREST FARM ("MCF") equine-related activities,including but not limited to clinics, practices, shows, competitions and related or incidental activities and MEDINA COUNTY FAIRGROUNDS. I acknowledge that there are certain inherent risks regarding equine activities, including but not limited to:

    • The propensity of an equine to behave in ways that may result in injury, death, or loss to persons on or around the equine;
    • The unpredictability of an equine’s reaction to sounds, sudden movement, unfamiliar objects, persons or other animals;
    • Hazards, including, but not limited to, surface or subsurface conditions;​
    • A collision with another equine, another animal, a person, or an object;
    • The potential of an equine activity participant to act in a negligent manner that may contribute to injury, death, or loss to the person of the participant or to other persons, including, but not limited to, failing to maintain control over an equine or failing to act within the ability of the participant.

    I hereby assume all risk of illness, injury or death to which I may be exposed to by participating in an activity or MCF event. ​

    I hereby discharge, waive, release from liability and hold harmless MCF, its successors and assigns, and any of its owners, employees and agents (Individually and Collectively, the "Released Parties" or "Event Organizers"), from any damages as a result of illness, injury or death to myself or any of my property as a result of my participation in an activity at any MCF event. This waiver and release from liability, and all terms contained herein, shall also be binding on my dependents, heirs, executors, administrators and personal representatives.

    By signing this Agreement, I (as the participant or as the Parent/Legal Guardian of the minor identified below) acknowledge that I have read and fully understand this Waiver and Release. Further, I understand that by signing this Agreement I am giving up certain legal rights including the right to recover damages in case of injury, death or property damage arising from my participation in an equine-related activity or MCF event.​

    I am further aware of the rules and regulations in effect by MCF and agree to abide by them, including any rules and regulations pertaining to the wearing of an equestrian helmet.

    SAFETY HELMET AGREEMENT


    Each and every individual owner, leaser, student, and handler, must wear an approved helmet at Maple Crest Farm approved events. Riders are encouraged to help each other to police this effort while catching, leading, grooming, riding, lunging, bathing, driving, exercising, showing, and training the horses.

    Our goal is to reduce the chances of a head injury due to the size and wight of the animals on this property. Horses are a herd animal, they are also prey animals, nature can sometimes takeover our domestication of equine.

    If riders cannot follow this rule, it is within MCF's rights to excuse the rider from the property.

     

    I HAVE CAREFULLY READ THIS DOCUMENT IN ITS ENTIRETY, UNDERSTAND ALL OF ITS TERMS AND CONDITIONS, AND KNOW IT CONTAINS AN ASSUMPTION OF RISK, RELEASE AND WAIVR FROM LIABILITY, AS WELL AS A HOLD HARMLESS AND INDEMNIFICATION OBLIGATIONS.

    The parties agree that this agreement may be electronically signed. The parties agree that the electronic signatures appearing on this agreement are the same as handwritten signatures for the purposes of validity, enforceability, and admissibility.
