I certify that the information provided on this form is correct, and that I hereby authorize Carolina Smart Home (BFD Carolina LLC) to initiate automatic deposits to my account at the financial institution named above. It is my responsibility to notify Carolina Smart Home immediately (1-800-750-3718, Ext. 701) if I believe there is a discrepancy between the amount deposited to my bank account and that reflected on my invoice. I also authorize Carolina Smart Home to make withdrawals from this account in the event that a credit entry is made in error.
Furthermore, I agree not to hold Carolina Smart Home responsible for any delay or loss of funds due to incorrect or incomplete information supplied by me or by my financial institution or due to an error on the part of my financial institution in depositing funds to my account. This agreement will remain in effect until Carolina Smart Home receives a written notice of cancellation from me or my financial institution, or until I submit a new direct deposit form to the Payroll Department.
Direct deposits only take place weekdays (except federal holidays and must be submitted to the banking institution before 4pm the previous business day), if this does not take place, Carolina Smart Home submit payment through Zelle or CashApp to avoid delays. If I decide to change my direct deposit information, I understand and agree that the terms of this authorization form remain in effect.
