It is our sincerest hope that by providing many of the Verification Day forms online, we may eliminate long lines to expedite your time on campus this day.

Please note that the sooner you submit, the sooner our staff can begin preparing for the day.  If you hope to pick up your Parking Permit, Uniform Purchases, and/or Athletic Passes on Verification Day, please complete this form by July 19 to be guaranteed that these items will be ready for pickup.

When submitting, please follow these guidelines:

• Use strong Wifi and make sure you are connected. We do not recommend using data.
• Use a computer—it seems to have trouble submitting when using a phone or iPad.
• Wait to be sure you get the thank you page. Sometimes it takes a few minutes to submit, however you should still get a thank you page at the end when the submission goes through.
• You should receive a confirmation email.  This will provide a link to edit.  If you do not receive this, your form did not submit.
• Do not hit refresh, as it will clear out what has been inputted and send you back to the beginning.

Health Services coordinates the State of Indiana programs for the prevention and detection of physical ailments. This includes administration of hearing tests for sophomores, and the maintenance of all medical records, especially immunization records.

This office is also responsible for distribution of medication, including those students under a doctor’s care. Students may not take medication or leave the building because of illness without authorization from the nurse and parents. Students are not permitted to carry medications; this includes prescription medicine and non-prescription (OTC – Over The Counter) medicine. All prescription and non-prescription (OTC – Over The Counter) medicine MUST be turned in to the nurse or the main office upon entering the school building. All medications MUST be in their original containers (no medicine will be accepted in plastic baggies). Additionally, all medications must be taken under supervision of the nurse or the front office personnel. Exceptions can be made for inhalers, epinephrine pens (EpiPens), cough drops, and eye drops but the proper forms MUST be submitted in advance.

If a student becomes ill in school, he or she should report the illness to a teacher and request permission to go to the nurse's office. The nurse will determine whether the parent/guardian should be notified and the student sent home. The student is NOT to text/call a parent from school without permission from the school nurse.

Pursuant of Indiana Code 20-34-4-2

(a) Every child residing in Indiana who is enrolled in an
accredited elementary school or high school shall be immunized as
determined by the state department of health against:

(1) diphtheria;
(2) pertussis (whooping cough);
(3) tetanus;
(4) measles;
(5) rubella;
(6) poliomyelitis;
(7) mumps;
(8) varicella;
(9) hepatitis A;
(10) hepatitis B; and
(11) meningitis.

(b) The state department of health may expand or otherwise
modify the list of communicable diseases that require documentation
of immunity as medical information becomes available that would
warrant the expansion or modification in the interest of public health.

(c) Before November 30 of each year, the state department of
health shall publish a two (2) year calendar of immunization
requirements and recommendations. The calendar must include:

(1) recommendations for immunization requirements for the
year subsequent to the following school year.

(d) The publishing time frame for the calendar described in
subsection (c) does not apply in the event of an emergency as
determined by the state health commissioner.

(e) The state department of health shall adopt rules under
IC 4-22-2 specifying the:

(1) required immunizations;
(2) child's age for administering each vaccine;
(3) adequately immunizing doses; and
(4) method of documentation of proof of immunity.

Saints Helping Saints

The Development Department is tasked with raising funds to defray tuition costs, as well as funding other school needs and projects. This can only be achieved with the assistance of many volunteers who share their time with us.

Each family benefits from our fundraising efforts, and thus every family is asked to contribute a minimum of 4 hours of their time, per student enrolled. There are many ways you can help and many hands make light work!

Please indicate the events and tasks that interest you. Please note that indicating your interests does NOT commit you to anything. As the dates of the events get closer, you will receive an email from Sign-Up Genius volunteer software with details and times, and at that point you will then be asked to commit to tasks or events that best fit your schedule.

Youth and Family Counselor

A full-time professional counselor is available for students, faculty, and staff through referrals from teachers or requests from parents. Students are helped with a variety of concerns including, but not limited to, depression, anxiety, and grief issues. If you prefer that your child(ren) not be allowed to talk to the counselor, please complete the information below.

First School Day for Freshmen:  August 7, 2024

A Freshman Mentor assists in orienting our new freshmen on their first day of school, August 7, and into the first week or two.

If you would like to serve BD in this manner, please sign-up below. Parents, please check your children's schedule before signing them up. The general expected duties are:

1. Be at Bishop Dwenger from 7 am until approximately 11:30 a.m. We will provide a small breakfast.

2. Lead a small group of 9th graders around the building, explaining BD procedures, answering questions and in general, making them feel welcome and comfortable.

3. Come with a positive attitude and be a great BD Ambassador.

4. Check your Bishop Dwenger email for messages from Mrs. Horne who coordinates this day. (firstname.lastname@bishopdwenger.com)

5. And finally, you agree to make welcome and encouragement signs to put on their lockers during the first 2 weeks of school.

You will receive National Honor Sociey, BD Ambassador or 5 Service Hours for helping.  

Students who are a returning BD Student or have used CANVAS at a previous school do not need to participate in the Canvas Learning Management Training.

Prepare to "Merge" into high school with an evening filled with fun, games, challenges, prizes, and refreshments!

"The Merge” will be held on Thursday, August 1 from 7:00 - 9:00 PM (please be prompt) while Freshman parents are attending the new parent meeting at Bishop Dwenger High School.  Sign up below for a head count.

Senior Students & Adult Volunteers

Student volunteers may earn NHS, BD Ambassador, or Flex Service Hours.  Event runs 7-9pm, student/parent volunteers need to arrive 6:30pm for a meeting and set up.  Adults are needed to assist at the various activities.  We would love to have your help to welcome our newest Saints!

We understand that the handbook contains important information about the school, its administration, and about the educational and disciplinary polices and procedures that the school maintains in furtherance of its religious mission as part of the Catholic Diocese of Fort Wayne-South Bend.

We agree to follow all rules and guidelines imposed in school by the school administration and/ or the Diocese. If we have any questions about the content of the handbook, we understand that it is our obligation to ask questions for clarification.

However, failure to read the handbook shall not relieve us of the obligation to follow all rules and guides that the school and the Diocese establish or in any way impede or prevent the school administration from operating the school consistent with those rules and guidelines.

Each student's parent/legal guardian, must acknowledge in writing that they have the handbook and have reviewed its contents with the student.  Diocesan Policy 4510 

It is highly encouraged for parents to review the handbook with their student.  At the start of the school year, following class meetings and another review of the handbook, each student will be asked to sign a pledge stating that he/she has read and understands the handbook contents.
