How to use this form: 

Checkbox Click the checkbox next to each item you wish to include in your order. This will add to your 'cart'.

Quantity Once the checkbox is selected, this item is included into your cart. Use the the drop-down menu by clicking the small arrow to edit the quantity you want to add to your cart. The default selection is quantity (1). 

Deselect Items If you decide not to include an item in your order, simply uncheck the checkbox next to the item name. This will remove the item from your cart.

Additional Information Fill in any additional required information, such as your contact details and delivery address.

Submit Click the "Submit" button at the bottom of the form to complete your order.

Confirmation You will be redirected to a confirmation page with the details of your order shortly after submission. If you do not see this page after clicking submit. Please contact us to confirm your order, otherwise we will contact you once we have confirmed your order and delivery fees.
