• Leap of Faith Jump Waiver And Release Form

    This is a legal document. By signing this document, I am agreeing to give up certain legal rights including the right to sue. I am also assuming certain obligations. My signature on this waiver means that I agree, warrant and covenant as follows:

  • • I am voluntarily participating in the Cyrus Centre Ministries “Leap of Faith” Fundraiser, an event sponsored by the Cyrus Centre Ministries, on Thursday July 11th, 2024, at Skydive Vancouver


    • I am aware of and appreciate the risks involved in a tandem skydive jump with a Skydive Vancouver instructor


    • I will sign appropriate waivers the day of the jump for insurance purposes for Skydive Vancouver


    • I am representing Cyrus Centre Ministries, and collectively representing and upholding the positive images of Cyrus Centre Ministries


    • I am physically capable of completing this event and I am solely responsible for my personal health and safety, and the personal property I bring with me


    • I understand that I must raise a minimum of $800.00 in pledges which are to be collected and submitted on or before Thursday, July 11th, 2024, and I am liable for such funds


    • I understand that failure to collect the funds for a pledged amount is my responsibility


    • I understand that all donations processed are non-refundable and non-transferable


    • I understand that all pledges are eligible for a charitable tax receipt issued by the Cyrus Centre Ministries when provided with a complete mailing address


    • I understand that my name, photograph, voice or likeness may be used for promotional purposes


    • I understand that I will not financially benefit from any use thereof


    • I further agree that my participation in the event is subject to the sole discretion of the organizers


    • I understand that my participation may have undesired and unanticipated consequences; notwithstanding this, I hereby release and forever discharge Cyrus Centre Ministries and all affiliates, members, directors, officers, leaders, agents,
    volunteers and employees from any and all actions, causes of action, suits, claims, demands, liabilities, including negligence, and expenses I have now or may have in the future in connection with, arising from, or related to my involvement with or
    participation in this activity.


    I have carefully read this release and fully understand its contents. I sign it of my own free will.
