Please carefully read the following instructions before completing this registration form:

General Information

1. 📅 Dates: Friday 4 October to Monday 7 October 2024
2. 📋 Mandatory Registration: Every participant must complete a registration form, whether staying overnight, or just doing day trips.
3. 📄 Waiver Forms: For participants under 18 years old, a waiver form must be signed by a parent/legal guardian.
4. 👕 T-Shirt Sizing: Sample T-shirts for sizing will be available at the Mandir Kothar.

Overnight Stay

1. 📋 Individual Forms: Each person requiring a bed must fill out a separate form.
2. 🏠 Accommodation Cost: The cost is $170 per bed for 3 nights, which includes an event T-shirt (males) or Shawl (females).
3. 🛏️ Bed Allocation: Each bed is suitable for one adult only.
   • 👶🏾 Infants: Enter details of infants sharing a bed with parents in the 'Bed Sharing' section.
   • 👥 Siblings: If siblings are able to share a bed, fill out a single form and enter the second sibling's details in the 'Bed Sharing' section.
   • Please note that Males and Females will be allocated in different rooms. However, kids under the age of 8 can choose to share room with either parent.
4. 👕🧣 Additional T-Shirts/Shawls: Only one T-Shirt/Shawl is included per bed. If your child is sharing a bed, please complete the 'Additional T-shirts/Shawl' section to purchase one for them. The cost for each additional T-shirt/Shawl is $30.

Day Trip Visitors

1. 📋 Individual Forms: A separate form must be filled out for each person making a day trip.
   • 👶🏾 Infants: Only infants aged 3 years and under can be included in the parent's form.
2. ☀️ Day Trip Cost: The cost is $20 per day per visitor. (Note: T-shirt/Shawl not included).
   • 👕🧣 Ordering T-Shirts/Shawls: To order a T-shirt/Shawl, complete the 'Additional T-shirts/Shawl' section.

In consideration for being allowed to attend and/or participate in this event, organised by Shree Swaminarayan Mandir Bhuj - Sydney Australia ('the Temple'), I hereby agree to the following terms and conditions:

1. Release of Liability: I hereby release from all liability and promise not to sue the Temple, including its employees and volunteers, from any and all claims, including claims of negligence, that may arise from my participation in this event.
2. Assumption of Risk: I am aware of the risks associated with this activity/event, including but not limited to the risk of physical injury. Nonetheless, I assume all related risks, both known and unknown to me, in participating in this activity/event.
3. Parental/Guardian Consent: If the participant is under 18 years of age, I confirm that I am the parent/legal guardian of the participant. I understand the legal consequences of this document, including the release of liability and assumption of risk, and I acknowledge that I am responsible for the obligations and acts of the participant as described herein.
4. Refunds: I understand that there are no refunds for this event once payment has been made. This includes but is not limited to cancellations due to personal reasons, changes in schedule, or weather conditions.
5. Transfer of Participation: Transferring my place in the retreat to another individual is acceptable only at the discretion of the organiser. If I wish to transfer my place, I must obtain prior approval from the event organiser.
6. Medical Consent: In the event of an emergency, I consent to the provision of necessary medical treatment for myself or the participant. I understand that I am responsible for any costs associated with such treatment.
7. Code of Conduct: I agree to adhere to the code of conduct established by the organisers and understand that any violation may result in my removal from the event without a refund.
