To help me organize the booking process, and help me get an idea of what you're interested in, please fill out any relevant information below. If there is a question you don't understand or are unsure of, give a best guess and let me know at the end how I can help.

Once this form has been filled out you will be automatically added to my waitlist, and from there you will be contacted for more information. Responses may take anywhere from 2 days to a week.

Placement Location

This is so I can get a better idea of where you'd like your tattoo on your body. This also allows me to fit your tattoo to your unique body shape to create a nice flow.

If there are scars, freckles, moles, etc. near where you'd like your tattoo, please take a well-lit photo of that area and some skin around it. Please note how old any scars are, and whether or not you would like them to remain visible or be covered in the description box.

Please note, any script in another language, I am unable to redesign or translate seeing as I can only read and write in English. Please make sure if it’s in another language the spelling and script is correct.
