By filling and submitting this form, you agree and understand that:
a) COOS may collect, use and disclose your personal data that you provide in this Form for the purpose of sending you updates about the workshop. You also consent to the disclosure of your personal data to COOS volunteers and other third party service providers that COOS may engage for this workshop.
b) If you are providing someone else’s personal data or submitting this Form on behalf of someone else, you hereby declare that you have obtained consent from the name individual(s) in this Form, for the collection, use and disclosure of his/her personal data by you to COOS, COOS volunteers and other third party service providers.
c) COOS respects the privacy of individuals and recognizes the importance of the personal data you have entrusted to us and believe that it is our responsibility to properly manage, protect, process and disclose your personal data.
d) COOS reserves the right to use any photograph/videography taken or at the workshop, without the expressed written permission of those included within the photograph. Photographs may be used in publications or other media material produced, used or contracted by COOS including but not limited to: view books, catalogues, search pieces, newspapers, magazines, television, reports, websites, etc. Any person desiring not to have their photo taken or distributed must contact COOS Administration Office (info@coos.org.sg ) in writing of their intentions and include a photograph. The photo will be used for identification purposes and will be held in confidence by COOS Administration Office.
