The presentation of this document is mandatory to be able to participate in training sessions and matches. In order for us to be able to process this application the following documents must be attached:

• I authorize the participation in training, matches, trips and activities related to regular competition and that are carried out both in his/her category and in higher categories with the Valencia Hockey Club. If both parents do not sign, the undersigned legal representative declares that they act with the consent of the other parent, exonerating the Valencia Hockey Club from all liability.
• Authorizations granted will be valid throughout the duration of the applicant´s participation in and connection with the Valencia Hockey Club. Likewise, please inform the club of any changes to the information provided, so that what the club has on file is correct and up to date and the club is able to contact you when appropriate.

VALENCIA CLUB DE HOCKEY is responsible for processing the personal information and data provided, with your consent, and informs you that this data will be processed in accordance with the provisions of Regulation (EU) 2016/679 of April 27 (GDPR) and the Organic Law 3/2018 of December 5 (LOPDGDD), whose purpose is the sports, economic and administrative management derived from relations with the Club. The data will be kept no longer than necessary for processing and when it is no longer necessary for that purpose, it will be deleted with adequate security measures to guarantee the pseudonymisation of the data or the total destruction of the same. Your data may be transferred to the Hockey Federation or other relevant organizations and when required by law. The VALENCIA CLUB DE HOCKEY is responsible for maintaining a personal data file, which includes the data of the players registered with the Club and their parents or guardians, if they are minors. Specifically, this file will include personal, economic data and, where appropriate, recordings of events and sports competitions or celebrations of the Club, as well as photographs.

The purpose of this file is to allow the club to track and monitor the performance of players, as well as club communication with them, locate parents and / or guardians when necessary, to allow the club to send information regarding the club, as well as registrar correct player information in official or extraordinary competitions in which the VALENCIA CLUB DE HOCKEY participates in or intends to participate in. By signing this clause, you consent, or in the case of minors, the guardian consents on their behalf, to the processing of your, or the minor´s, personal data and images and to their (with the exception of specially protected personal data) being published on the club's website and / or social networks, and/or in publications, interviews or broadcasts, whose goal is to facilitate or celebrate your, or the minor´s, participation in sporting or socio-cultural events as a club member.

VALENCIA CLUB DE HOCKEY may transfer your data (with the exception also of specially protected personal data) to Associations, Official and Unofficial Organizations, Public Administration, Federations and other Sports Clubs or Associations for the purpose of participating in competitions and events. The club also advises you of your right to not consent to the processing of data about your ideology, religion or beliefs. The club understands that by signing this clause, you DO NOT grant this consent unless otherwise expressly stated in writing.

In the event that you provide the VALENCIA CLUB DE HOCKEY with personal data of third parties (family, partner, friends, etc.), by signing this clause, you guarantee that you have previously informed the third party of your intention to share their information and that you are authorized to do so, under the same conditions and for the same purposes set forth in this document.

VALENCIA CLUB DE HOCKEY informs you that you or the minor you represent may be required as club players to undergo, as appropriate, a series of prior medical examinations in order to insure and federate you or the minor you represent. Therefore, the club requests your express consent to obtain and process your health data, in order to know your ability to participate as a player in official and unofficial competitions, as well as to monitor your health during your stay in the club where appropriate, guaranteeing maximum confidentiality in the treatment of the same.

The VALENCIA CLUB DE HOCKEY informs you that, unless you object, the club can publish your name and images on the club's website and / or social networks. VALENCIA CLUB DE HOCKEY guarantees the confidentiality of the personal data provided and in response to your concern to guarantee the security and confidentiality thereof, has adopted the required levels of security for the protection of personal data, and has installed the technical means to within its means to prevent loss, misuse, alteration, unauthorized access and theft of personal data provided to the VALENCIA CLUB DE HOCKEY.

The authorizations granted will be valid throughout your or the minor you represents´ connection with and participation in the VALENCIA CLUB DE HOCKEY activities. Specific authorizations for image and sound treatment: The interested party authorizes the capture of and reproduction of images and sound during the participation in or presence in competitions, activities, events, ... organized by the club or third parties, for internal and external purposes for training and promotion of the VALENCIA CLUB DE HOCKEY.

Data communication authorization: The interested party authorizes the communication of data to third parties to manage registrations in competitions, activities, events, ... as well as managing collection of Club fees by direct debit.

Remember that:

1) Attendees at events that capture images of players via video, photography or any other digital means need to be aware that their use must be exclusively personal, never commercial. And that these types of recordings and photographs should not be published or make public unless you have the express consent of ALL people that appear in the images, and in the case of minors, the consent of their parents or guardians.

2) In case both parents do not sign, the parent / legal representative, declares that he / she acts with the consent of the other, exonerating the Valencia Hockey Club from all responsibility
