Albury's Advance Athletics Liability & Media Release Waiver

I am First Name Last Name the parent/legal guardian of athlete First Name Last Name hereby affirm that I am voluntarily starting a course of instruction in physical fitness and performance training with Trevarus Albury, Albury's Advance Athletics LLC, and or its other trainers, and associates. I am voluntarily participating in the training program entirely at my own risk.

In full consideration of the risk of injury while participating in the Activity, and for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily participate in this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any kind of risks related to traveling to and from as well as participating the Activity, which may include, but are not limited to, physical or psychological injury, pain, suffering, illness disfigurement, temporary or permanent disability, economic or emotional loss, and death.

Media Release Clause

I hereby grant permission for my child to be photographed and filmed by videographers, photographers, and drone operators during the period of Coach T's training sessions. I understand that these images and videos may be used for promotional purposes on social media and the official website. I waive any rights to image likeness or compensation for my child's appearance in these materials and release Coach T's Baseball Camp and its affiliates from any claims related to the use of these images and videos.

Refund Policy

Please note that all training sessions are final. We do not offer refunds for any reason, including but not limited to, personal conflicts, injuries, or changes in schedule. By enrolling your child in one on one, group training session, or camp, you acknowledge and agree to this no refunds policy. You may rescheulde your session one time with a 24 hour notice. Thank you for your understanding and cooperation.

I acknowledge that I have carefully read this form and fully understand that it is a release of liability. I expressly agree to release and discharge of Trevarus Albury, Albury's Advnace Athletics LLC, The Elite Factory, and it's trainers or associates from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I may otherwise have to bring a legal action for personal injury or property damage.
