Purpose of the Consent

As part of our pre-employment screening process, Parham Trucking Empire LLC requires all potential employees to consent to a Motor Vehicle Records (MVR) review and drug testing. This process is essential to ensure the safety and integrity of our operations.

I understand that the information obtained will be used solely for the purpose of determining my suitability for employment and will be kept confidential. I also understand that any false or misleading information provided by me during the application process may result in disqualification from employment or termination if already employed.

I hereby release Parham Trucking Empire LLC, its officers, employees, and agents from any liability or damage that may result from the use of such information.
