    PRE-FLYING TABLES CHECKLIST

  • INSPECTION ITEMS

  • 1. PSI completed and signed off by all workers and foreman. Any new workers to task review and sign off JHA.*
  • 2. Fall protection and certified anchor points in place. All necessary components gathered/inspected (column straps, SRLs, etc.)*
  • 3. Controlled Access Zones (CAZ) in place. Leading edge work areas and ground level below tables are barricaded with proper guardrails/signage. Smart edge/2x4’s staged on bottom level.*
  • 4. Walking paths set up for workers outside of CAZ with barricades/crowd barriers. Proper access ladders in place.*
  • 5. All extra/loose material and concrete debris removed from top and bottom deck work areas and beam flanges. Decks are swept clean. Loose material stored on table joists removed.*
  • 6. All filler material (4x4s, plywood patch pieces, etc.) at the outside perimeter are secured to the table.*
  • 7. Inspect compensator, spreader bar, winch, turtles, tanks, and remotes before task start.*
  • 8. Inspect rigging at compensator, picking points, taglines, shackles, hooks, and ensure bolts are tight before task start. Rigger is qualified.*
  • 9. Inspect all formwork and table components before task start, cross-check with engineered drawings and ACS specifications. Table jacks set within allowable tolerance.*
  • 10. Inspect chain falls. Ensure chain falls in place between truss and nearest column on both sides of table as back-up safety device.*
  • 11. Crane operator, spotter, top and bottom level foremen all have operational radios before task start.*
  • 12. Spotters in place and have airhorns.*
  • 13. Pre-fly meeting held. All trades onsite aware of table operation.*
