I realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of all spaces used by Anvita Sharma and Nrityanvita Dance Academy. I am also ensuring I or my child are in the proper physical condition and capable of participating in these activities. 

I agree to release and hold harmless Anvita Sharma and Nrityanvita Dance Academy including its teachers, dancers, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Anvita Sharma and Nrityanvita Dance Academy liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Anvita Sharma and Nrityanvita Dance Academy. 

I understand that Anvita Sharma and Nrityanvita Dance Academy are licensed and insured. I also understand Anvita Sharma is not responsible for my child or other children who are left unsupervised and that she will only be supervising my child when he/she is participating in the scheduled dance activities or programs. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to Anvita Sharma, instructor or staff member as soon as possible. 

I hereby authorize Anvita Sharma and Nrityanvita Kathak Academy to publish the photographs and videos taken of me and/or the undersigned minor children, for use in Nrityanvita Kathak Academy promotions on social media, website and advertising materials. 

I release Nrityanvita Kathak Academy from any expectation of confidentiality for the undersigned minor children and myself and attest that I am the parent or legal guardian of the children listed below and that I have the authority to authorize them to use their photographs and/or videos. 

I acknowledge that since participation in publications, social sites and websites produced by Nrityanvita Kathak Academy is voluntary, neither the minor children nor I will receive financial compensation. 

I further agree that participation in any publication and website produced by Nrityanvita Kathak Academy confers no rights of ownership whatsoever. I release Nrityanvita Kathak Academy, its contractors and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.
