In consideration of being allowed to participate in any way in any WalkGood LA (also known as "WalkGood Inc.") yoga, health, wellness program, or related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, and assume full responsibility for my participation; and,
3.MEDIA RELEASE: I give WalkGood LA non-revocable permission to capture my image and likeness in photographs, videotapes, motion pictures, recordings, or any other media (collectively "images" WalkGood LA will own the images and be granted permission to copyright, display, publish, distribute, use, modify, print and reprint such images in any manner related to WalkGood LA, and/or its related business, including without limitation, social media, marketing platforms, publications, promotions, advertisements, brochures, website, or other electronic displays and transmissions. I waive any right to inspect or approve the use of the image by WalkGood LA prior to its use, and additionally waive any right to any profit WalkGood LA may obtain from the use of the aforementioned images. I forever release and hold harmless WalkGood LA from all liability arising out of the use of the images in any manner or media, and waive all claims and causes of action relating to use of the images, including without limitation, claims for invasion of privacy rights or publicity; and,
4. I understand any items, personal belongings I bring to a WalkGood LA experience/activity are my sole responsibility, and WalkGood LA is not responsible for lost, missing, stolen or damaged items; and,
5. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
6. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS WalkGood LA (also known as "WalkGood Inc.",their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, board members, volunteer, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property or personal property and items, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
