St. Mary Catholic Athletic Participation Form

The following information must be completed and signed by the appropriate parent or guardian and turned in to the main office before participation in student athletic activities will be allowed.  If the following information is not complete, this form will be returned to you.  All students participating in student athletic activities at St. Mary must have their own medical coverage. Students will not be allowed to participate in student athletic activities unless the following information is submitted and the form is signed by the parent or the guardian of the student.

We, the undersigned hereby certify that I am the parent or legal guardian of the student.  I hereby give permission to the staff of St. Mary Catholic School to seek during the period of school athletic activities, appropriate medical attention and for the students to receive medical attention and treatment to be covered under the student’s insurance policy detailed above.  I/We the undersigned, for ourselves, our heirs, our executor and administrator, waiver, release, and forever discharge St. Mary Catholic School and its staff, officers, agents, employees, representatives, successors and assigns from any and all liability claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, personal injury or property damage that may be sustained or occur during participation in student athletic activities or while at school.

I give my permission for my child to ride with other team parents during the 2024-2025 school year.  I understand these parents will be driving their own private vehicles.  St. Mary School assumes no responsibility for the driving performance of these parents. The parent driver is responsible for all automobile insurance coverage in case of an accident while driving students to and from games. Students will leave for away games from St. Mary School.In consideration for my child’s participation on this sports team, I agree to release and hold harmless from any legal liability and agree not to sue St. Mary Catholic School, its employees, agents, and representatives for any injuries, death, or personal property damage resulting from transportation by a parent volunteer to or from a competition.

The fee for athletics will be due before uniforms are issued for each sport. The fee will help to defray the costs related to the sport (i.e. referees’ fees). Any family who has a child who would like to participate in St. Mary Athletics but who cannot pay the fee can talk to the Athletic Director about scholarship opportunities.
