The property owner, and or property representative, signing this form does hereby give Suburban Whitetail Management LLC (SWM) permission to archery hunt on their property. The activity on the property will be conducted in full compliance with all Pennsylvania state game laws, and SWM rules and regulations.

This permission is valid for any and all future whitetail deer hunting seasons and may be terminated by either party at any time by written notice.
