Tattoo Appointment Request Form

Please use the following form to submit a tattoo design. Once your request has been reviewed and you are eligible to schedule, you will receive a thorough text message with a price quote and how to proceed with booking. Please be patient as forms are reviewed on a biweekly basis.

A non refundable booking fee is REQUIRED to book your appointment ($50-$100) and goes toward the total quoted price of the tattoo. Once invoiced, I require 48 hours to have this paid or the appointment will be released.

Friday/Saturday availability is limited. If this is your only availability, it may be a longer wait to get scheduled. I don't take appointments past 3 pm.

You can submit multiple pieces per appointment (if you are unable to fit them in this form, please let me know once I reach out to schedule)

If you don't have a solid idea your request will not be accepted, please be as specific as possible. Your appointment is accepted based on design approval. Changing your design could result in a cancelled appointment if the design does not suit my style/portfolio. If you’d like to change your design you must text me a minimum of 1 week before your appointment. Thank you for choosing me for your next tattoo! I look forward to working together.

References - A can of coke is 4.8 inches tall, a business card is typically 2.5 inches wide and a half dollar coin is about 1.20 inches in diameter.

Please take a photo of the body part you want tattooed. This should be taken straight on in good lighting of YOUR body part especially for larger projects. (I.e. forearm, shoulder, side of knee etc)*

Please provide a font name for script tattoos (this does not apply to personalized handwriting) DO NOT post a picture and say “similar font”. Please find the font you want and list here.

Please state your budget (if any). I will do my best to work with your budget within reason. My tattoo MINIMUM is $150 meaning no matter how simple/small the design is. I charge a FLAT RATE for tattoos. Please request a quote estimate prior to booking an appointment. *

Policies
1. All appointments require a non refundable booking fee to secure an appointment. Once approved for booking, you will receive an invoice via square in a text message. This fee MUST be paid within 48 hours of receipt or the appointment will be forfeited.

2. Cancel/reschedule/no show: Cancellations or rescheduled appointments with less than 72 hours notice, will lose their booking fee and require a new one to reschedule. Being over 15 minutes late for your appointment may require the need to reschedule at my discretion. You are allowed to reschedule your appointment ONCE given 72+ hour notice. Same day cancellations will forfeit their booking fee and be required to pay 50% of the service missed in order to reschedule. No shows will forfeit their booking fee and you will NOT be rebooked unless a new booking fee and 100% of missed service is paid.

3. ONE guest is allowed. Showing up with guests without informing me may result in your appointment being canceled and booking fee forfeited. If you are getting matching tattoos, DO NOT bring additional guests. I have a small treatment space and it is distracting/overwhelming to have too many people in my room.

4. Fine line tattoos may require a touch up due to their delicate nature. I offer one free touch up as long as it's within 6 months of your tattoo appointment. Touch ups can be performed a minimum of 4 weeks after the initial appointment. Any additional touch ups will require a fee. Failure to follow proper aftercare may result in unsavory healing/retention.

5. NO REFUNDS are given under any circumstance.

6. I reserve the right to refuse any client that is not appropriate for this procedure.

I understand that special circumstances are unavoidable and it may be necessary to cancel your appointment. If you find yourself unable to keep your appointment, I request 72 hours notice. Same day cancellations will require 50% of the service missed to be paid in order to reschedule. Failure to show will have a 100% service fee required to rebook. Cancellations OR rescheduled appointments with less than 72 hours notice, will lose their booking fee and require a new one to book in. Being over 15 minutes late for your appointment may require the need to reschedule at the artists discretion. If so, you will lose your booking fee, and will require a new one to rebook.

You are allowed to reschedule your appointment ONCE if given 72+ hour notice. More than once and your deposit will be forfeited and a new fee must be paid to rebook.
